NHS Trafford Clinical Commissioning Group is looking to commission a service which will help support the delivery of integrated care for Trafford patients.
In an integrated care system the patient will be seen by the appropriate team/clinician at the right place. This is about taking the service to the patient; treatment may be in the patient's home which may for many be a nursing or residential home, a GP surgery, at a hospital or in the community. This new way of coordinating care in Trafford will be through a patient Care Coordination Centre. The Principles, Infrastructure, Core Services/Deliverables, and Benefits/Outcomes are described below.
Principles
- A Health and Social Care 'proactive system';
- Patients should always get the right care at the right place at the right time, through an effortless journey;
- A focus on complexity and vulnerability regardless of age.
Infrastructure
- Single phone point / one point of contact;
- Single 'live' directory;
- Supported by IT infrastructure - explore use of technology i.e. mobile messaging reminders;
- IT infrastructure to capture and translate data;
- Access to all records i.e. patient care plans feeding in to enable proactive planning;
- Alignment to 111 and out of hours;
- 7 days a week service. Night - probably continue with data/info service rather than a manned service;
- Virtual or physical centre? To be explored via competitive dialogue process.
Core Services/Deliverables
- Tracking of patient journey;
- Close monitoring of vulnerable patients i.e. expected discharge date from acute following through to community services/primary care;
- Links to transport services;
- Meaningful links between transport and appointments (relationship);
- 'Auto pick up' patients as they go through the system. Patient is in the system from the point of admission onwards;
-- Clinical triage to support monitoring of patient and appropriateness of care delivered to need;
- Brokerage - Give Options I.e. Care Planning/personal Budgets. Trigger/criteria To Access This?

A Prior Information Notice (PIN) was published to the market via OJEU; in relation to the competitive dialogue procurement process Trafford Clinical Commissioning Group (Trafford CCG) will shortly be undertaking to facilitate the development of a Care Coordination Centre in Trafford. Following publication of the PIN; NHS Trafford CCG held a Bidder Day for prospective suppliers to attend to recieve further information in relation to this opportunity. The event took place at 9:30 am on Friday 7th June at the Life Centre, 235 Washway Road, Sale, M33 4BP.
Following this event, Trafford CCG has continued to refine its approach to the competitive dialogue procurement of the Trafford Care Coordination Centre, with a view to publishing a contract notice in August 2013.
Prior to publication of the contract notice, Trafford CCG would like to offer interested prospective suppliers the opportunity to engage in an additonal information session. The purpose of this session would be to allow a valuable exchange of information between Trafford CCG and prospective suppliers; the outputs from which will potentially be used to further refine the Trafford approach to care co-ordination, as well as supporting suppliers understanding in terms of the proposed service.
Please note that all suppliers will be welcomed to participate in this pre-PQQ session, regardless of whether they have previously expressed an interest or attended the bidder day.
Trafford CCG is flexible in terms of the best way to structure this session; the precise format will be finalised and communicated to interested suppliers once we have an idea of the level of demand. Should a 'face to face' format be considered most appropriate, this will take place on one of the folllowing days: 24th July; 25th July 26th July.

If you would be interested in participating in this pre-PQQ additional information session please register your interest with Ellie Tivinan ( +44 1618739697) no later than 16:00 pm on Wednesday 17th July. Following this date, Trafford CCG will take a final decision on the most appropriate format for this information session, and this will be communicated to all providers along with a confirmation of the date and venue, if applicable.

A template will be issued to each supplier who registers an interest, and we will ask that this is completed with a list of questions to which providers would like Trafford CCG to respond. The Trafford CCG response to these questions will be fed back to all suppliers as part of the information session.
Please note that any information exchanged as a result of this pre-PQQ information session will be published in full as part of the PQQ documentation issued with the contract notice.
