2. Continuing Care for Children and Young People including PHBs.

II.2.4)Description of the procurement:

The key aims and objectives include meeting the requirements of the following:

- the National Framework for NHS CHC and NHS FNC (revised 2012);

- the National Framework for Children and Young People's Continuing Care (revised 2016);

- National Guidance and regulations regarding the implementation of PHBs including the NHS Direct Payments Regulations (Amended 2013);

- Children and Families Act 2014;

- The SEND Code of Practice January 2015;

- Act as the health case manager for the reablement service for those individuals;

- Assessing patients who have been identified as likely to be eligible for CHC, or who are placed in a nursing home and are eligible for receipt of FNC;

- Reviewing those patients in receipt of fully funded CHC, including fast track funding, joint funded care packages, or FNC;

- Assessing children and young people who have been identified as likely to be eligible for continuing care including those in transition.
